To take an appointment you must come in person to the Clinic front desk with your request, at Ash or Centre Street.
Before coming to the Clinic for your blood test, you must make sure that the following information are written legibly on each request:
- your name,
- contact information,
- health insurance number,
- date of birth,
- doctor’s name, license number and fax number.
Any non-compliant application will be refused. Contact your doctor or nurse to correct or add missing information.
Don’t forget your request when you come for you appointment!
If you don’t attend your appointment without notifing us in advance, we won’t be able to give you a new one.
You will have to go to another walk-in clinic without an appointment.
