You have a complaint to make?

We encourage you to read the code of ethics of the Clinic to know Our commitment, your rights, your responsibilities, and how to go about making your recommendations or lodging your complaint.

If you are not satisfied with the services you have received at the Clinic, we urge you to speak up. That way you will help improve our services. 

First, we encourage you to speak to the staff involved or to the person in charge of the service often they will provide a satisfactory response to your complaint. 

If you are not satisfied after taking this first step, you may contact the Office of the service quality and complaints commissionner:

By phone: 514 593-3600 If the commissioner does not answer, please leave a message that includes your contact information. 

By writing:

1: Download and print the complaint form (paper copies available at reception desk of the Clinic)
2: Complete, date and sign the form.
3: Place the completed form in a sealed envelope.
4: Give the envelope to person at the reception desk of the Clinic or send it by mail:
